Please use the VIDEX application forms for your visa application.

When filling out the application forms, please note the following information:

• The videx application form has to be filled in completely in English/German, printed out and signed on all pages.
• If you have only one name (no distinction between first name and surname) you need to write the same name in the chapter first name as well in the chapter surname.
• As type of reference a German address or the address of the first acommodation in Germany must always be given.
• ADDITIONALLY to the application, you need to print out and sign this declaration
