Please use the VIDEX application forms for your visa application. Only in case of technical difficulties the pdf „Schengen visa application form“ may be used.

When filling out the application forms, please note the following information:

• The videx application form has to be filled in completely in English/German, printed out and signed on all pages.
• If you have only one name (no distinction between first name and surname) you need to write the same name in the chapter first name as well in the chapter surname.
• As type of reference a German address or the address of the first acommodation in Germany must always be given.
